When to come in, and when to go somewhere else first

Most musculoskeletal pain is worth an exam once it has outlasted a week of sensible self-care, or sooner if it is stopping you sleeping, working, or driving comfortably. Waiting is not virtuous. The cases that take longest to resolve are usually the ones that were left for three months on the theory that they would sort themselves out, by which point the original problem has been joined by compensations in the joints above and below it.

There is a short list that should go to urgent or emergency care rather than to a chiropractic table, and Ken would rather you knew it before you book. Loss of bladder or bowel control, numbness through the saddle region, or rapidly worsening weakness in a leg needs an emergency department the same day. So does significant trauma with an inability to bear weight, or new severe pain alongside fever, unexplained weight loss, or a history of cancer. Chest pain or a sudden worst-ever headache is a medical emergency and not a spinal complaint until a doctor has said otherwise.

Short of that list, the exam is the deciding step rather than the phone call. Plenty of what walks in turns out to be mechanical and responds well. Some of it does not, and the useful outcome of a first visit is sometimes a referral to a physician, imaging, physical therapy, or a specialist. You are not charged more for the answer you did not want, and Ken would rather send you to the right place in week one than treat you unsuccessfully for six.

What the first few weeks usually look like

After the exam you get a plan with a number attached: how many visits Ken expects this to take, what should be measurably different by when, and what the re-check will look at. Uncomplicated mechanical cases often turn inside two to four weeks. Longstanding problems, disc involvement, and injury cases run longer, and you will be told that up front rather than discovering it at visit twelve.

Visit frequency starts higher and tapers. That is not a sales structure, it is how irritated tissue behaves — early on the effect of a treatment fades before the next one, and as the joint holds its correction the gap between visits stretches. If your gap is not stretching by the re-check, that is a signal the plan is wrong, and the plan changes.

Homework is part of it. Two or three specific movements, done most days, matter more to the durability of the result than any single adjustment. The patients who keep their gains are almost always the ones who did the unglamorous five minutes rather than the ones who came in most often.
